Thanks to visitors to the site, we've procured some of the forms employers are asking their employees to complete. We've included them below with some suggestions on how to deal with questions that are clearly out of bounds. Because there's no standardization; some are 4 reasonable questions, others are an inquisition and an invasion of privacy, it's very difficult to address every possibility unless we have examples.
If your employer is clearly looking for loopholes to deny your exemption, we suggest that you find an attorney in case you need to make an appeal. There are several firms listed in Additional Resources.
