It is not something you request. It is something that you inform your employer that you require. Federal law upholds exemptions based on religious belief. Your employer must make an accommodation unless it's a hardship to the company. And they must be able to prove hardship.
To file a Religious Exemption only requires a "sincerely held belief". You do not have to belong to a traditional faith to have a sincerely held belief that would preclude vaccination but your beliefs must be sincere and preclude taking the Covid shot.
A SINCERELY HELD BELIEF is all that is required by Arizona Law (ARS 23-206) and the EEOC Title VII.

Even those with non-traditional beliefs can apply for a Religious Exemption
U.S. v. Seeger was a pivotal case. In that 1965 case, the Supreme Court decided that a religious belief, even if it was unorthodox and nontheistic (no God), was genuinely religious if it occupied “a place parallel” to that of a traditional believer. According to this theory, everyone is or at least has the capacity to be religious—and religious beliefs are not about their content so much as how they are believed—deeply, sincerely, religiously.
Each employer must have a mechanism for accepting religious exemptions. Some are doing an online question and answer, some a printed question and answer and others are accepting a statement. A few are doing personal interviews. You can prepare and have notes.
There are 3 elements required to get an exemption:
1. You must clearly communicate your sincerely held belief.
2. You must clearly explain why that belief conflicts with the Covid shot.
3. You must clearly communicate an acceptable accommodation.
